In an effort to open a warranty claim on your behalf with the manufacturer of your bed, please complete the following form. This claim will be active for the next 30 days. If we do not receive this form back from you within 30 days, we will assume that you no longer wish to pursue this issue.

Please complete each field as completely as possible so we may present the entire document to the manufacturer and act as your advocate in our attempt to gain their approval for an exchange under the terms of their warranty. You may find it helpful to reference the warranty card that came with your mattress.

After we receive this completed form from you, our management will review it and see if it is necessary to send an Independent Bed Check service to your home. The service will then determine if your complaint is covered under the manufacturer’s warranty.
